By disagreeing on a counseling statement that I signed, do I still have to do the plan of action?

You are obligated to comply with the plan of action to the best of your ability. Your disagreement with the counseling session does not negate compliance with the plan of action. It simply documents that you do not agree with some element of the counseling (facts, tone, plan of action, etc).
